Dental Medicine Requirements for GCU Students

This serves to record the understanding between Grand Canyon University (GCU) and the College of Dental Medicine-Arizona (CDMA) at Midwestern University's (MWU) Glendale campus. Beginning with the class matriculating in fall 2015, CDMA agrees to guarantee interviews and reserve 5 acceptances, in total, for qualified GCU applicants per admission year. Applicants must meet all the requirements set forth below for reserved seating consideration. If by March 1st of the annual admissions cycle MWU has not accepted at least 5 applicants from GCU, then MWU may, at its discretion, offer the remaining seats for that cycle to other applicants. The reserved seats for GCU students will be reviewed annually with a potential adjustment to the number of seats depending on the total applicant pool from GCU and the educational progress of students admitted under this articulation agreement.

GCU students applying for admission to CDMA must satisfy the following minimum requirements before the academic year commences for the incoming class:

Achieve a minimum of a 3.50 cumulative grade point average (on a 4.00 =A scale) in their undergraduate coursework (with a 3.60 cumulative grade point average in sciences);
Have completed the minimum number of the prerequisite courses (as stated in the Midwestern University Catalog) in the prescribed subject areas at a regionally accredited college or university with a grade of C or better before the program begins;
Have the ability to successfully fulfill all technical standards for CDMA;
Receive a score on the Dental Admissions Test (DAT) within the three years prior to anticipated matriculation at MWU, commensurate with the average DAT score of the D-1 class from the previous year;
Receive a score of "High" from each of the CDMA interviewers on the day of admissions interview;
Demonstrate a good understanding of dental medicine and a sincere interest in a career in the field, including shadowing at least one general dentist prior to admission;
Dentally-related experiences that indicate sufficient exposure for candidates to make informed decisions about dental careers;
Personal integrity and sound moral character;
Reflect a people/service orientation through community service or extracurricular activities;
Abide by the Midwestern University Drug-Free Workplace and Substance Abuse Policy;
Pass a criminal background check; and
Meet all other requirements set forth by MWU, including requirements documented in the catalog, student handbook, and this agreement.

International Students

Must complete a minimum of 30 semester hours of coursework in the United States. Of the 30 semester hours, 15 hours must be in the sciences, 6 hours must be in non-remedial English composition, and 3 hours in Speech/Communication. Additional requirements for international students that have been accepted into CDMA are included in the matriculation agreement.

Final Decision

Applicants must meet all the requirements set forth below, as well as excel in their interview with MWU's Admission Committee, for reserved seating consideration. GCU students applying for admission to the PA Program in Glendale, AZ must satisfy the following minimum requirements before the academic year commences for the incoming class:

Achieve a minimum of a 3.60 cumulative grade point average (GPA) and a minimum of 3.70 cumulative science GPA (on a 4.00 = A scale) in their undergraduate coursework;

This Agreement does not guarantee or promise any Applicant acceptance or admission to any program at MWU, including but not limited to, the PA Program. This Agreement is for the benefit of MWU and GCU only and is not for the benefit of, nor may any provision be enforced by, any other person or entity, including any Applicant or student.

The PA Program will work with GCU's Office of Communications and Marketing (or its equivalent) to create marketing literature supporting the collaboration discussed in this Agreement. Each party will obtain written authorization from the other party prior to use of the other party's or any of its affiliate's name for any purpose. GCU will provide drafts of all promotional materials to the PA Program for review and approval prior to use or publication. This Agreement is not to be deemed exclusive, and either party shall be free to enter into similar agreements with third parties.

Optometry Glendale Requirements for GCU Students

This serves to record the understanding between Grand Canyon University (GCU) and the Midwestern University Arizona College of Optometry (AZCOPT) beginning with the class matriculating in Fall 2015. AZCOPT agrees to offer interviews and reserve 2 acceptances for qualified GCU applicants each admission year. Applicants must meet all the requirements set forth below, as well as perform acceptably in their interview with the AZCOPT Admissions Committee, for reserved seating consideration. If, by February 1st of the annual admissions cycle, AZCOPT has not accepted at least 2 qualified applicants from GCU, then AZCOPT may, at its discretion, offer the remaining seats for that cycle to other applicants. The number of reserved seats for GCU students will be reviewed annually and may be adjusted depending on the total applicant pool from GCU and the educational progress of students admitted under the articulation memorandum of understanding.

GCU students applying for admission to AZCOPT must satisfy the following minimum requirements before January 1st of the academic year for the incoming class:

Achieve a minimum of a 3.30 cumulative and science grade point average (on a 4.00 =A scale) GPA in their undergraduate coursework;

Of the 30 semester hours, 15 hours must be in the sciences, 6 hours must be in non-remedial English composition, and 3 hours in Speech/Communication. Additional requirements for international students that have been accepted into the MWU CVM Program are included in the matriculation agreement.

Final Decision

The final decision on acceptance for applicants shall be up to the discretion of the MWU CVM Admissions Committee, subject to approval by the Dean.

Although we anticipate a long and mutually satisfying relationship between our two educational institutions, this agreement may be discontinued for any reason providing that written notice is given 12 months prior to such discontinuation and that those students who have been accepted under this agreement be permitted to complete their course of study.
