Matriculation Agreement
This serves to record the understanding between the Physical Therapy (PT Residential and PT Hybrid) Program in the College of Health Sciences (CHS) at Midwestern University’s (MWU) Glendale campus and the Biomedical Sciences Programs in the College of Graduate Studies (CGS) on both the Glendale and Downers Grove campuses.
Eligibility Requirements
The MWU PT Glendale Program agrees to reserve a minimum of 10 seats for qualified Biomedical Science Program applicants per admission year. To be guaranteed a seat, applicants must meet all the requirements set forth below.
- Complete the Biomedical Sciences program at CGS of Midwestern University with a cumulative science grade point average of 3.25 with no failures in the Biomedical Science Program courses.
- Perform above average on interviews at PT Glendale Program.
- Demonstrate a good understanding of physical therapy and a sincere interest in a career in the field.
- Meet all other requirements and prerequisites set forth by MWU as documented in the catalog, student handbook, and this agreement, including but not limited to prerequisite courses, passage of a criminal background, and meeting the technical standards.
- Complete the CGS Biomedical Science program in which the student is enrolled by the summer quarter prior to the start of classes at PT Glendale Program. If the program is extended, even for thesis-only, the admission to PT Glendale Program will be rescinded. Students who enter a leave or withdraw from the CGS program in which they are enrolled are ineligible to retain admission under this agreement.
- Remain in academic and professional good standing throughout the student’s time as a Midwestern University CGS student. Admissions offer will be rescinded for students entering academic probation or disciplinary warning or probation.
Application Details
Applications from currently enrolled Biomedical Sciences students who apply will be processed as follows:
- Biomedical Sciences students who have achieved a minimum of 3.10 science grade point average by the end of fall quarter will be permitted to schedule an interview which will generally be offered by the December break.
- Students must achieve a minimum of 3.25 by the end of winter quarter of the application year to be eligible for guaranteed admissions.
- Students must maintain the 3.25 GPA through the remainder of the program and complete all requirements listed above to maintain their admission.
If by May 1 of the annual admissions cycle, PT Glendale Program has not accepted at least 10 applicants from the CGS Biomedical Science Program, then PT Glendale Program may, at its discretion, offer the remaining seats for the cycle to other applicants. The reserved seats for the Biomedical Science students will be reviewed annually with potential adjustment to the number of seats depending on the total applicant pool and the educational progress of students admitted under the matriculation agreement.
This agreement may be discontinued for any reason providing that written notice is given 12 months prior to such discontinuation and that those students who have been accepted under this agreement be permitted to complete their course of study. PT Glendale Program may recommend students to CGS Biomedical Science Program based on previous application to PT Glendale Program to improve student study skills.
This Agreement is intended for the benefit of the parties hereto only, and is not for the benefit of, nor may any provision hereof be enforced by, any other person or entity, including the student-applicant.
Note: Students must meet all application deadlines for the professional program of interest. A guaranteed interview does not guarantee admission into the professional program
